E2437. Complex Cystic and Solid Masses of Breast: Diagnostic Approaches and Various Disease Conditions

Background
According to BI-RADS 5th edition, the complex cystic and solid masses in breast ultrasound are described as “a complex mass containing both anechoic (cystic or fluid) and echogenic (solid) components”. A wide range of various diseases show these findings, and both benign and malignant diseases are included. Masses showing these findings are classified as Category 4 and require additional biopsy, and the possibility of malignancy is reported variously. Therefore, when evaluating these complex cystic and solid masses, other characteristic findings of the mass on ultrasound should be considered along with clinical information.

Educational Goals / Teaching Points
Approach of the complex cystic and solid masses in breast, clinical history and presentation, classification of the solid or cystic portion character, associated findings, shape, orientation, margin, posterior features, calcifications, associated features, vascularity, elasticity, categorization, and biopsy.

Key Anatomic/Physiologic Issues and Imaging Findings/Techniques
Benign condition of the complex cystic and solid masses, hematomas, fat necrosis, abscesses, granulomatous inflammation, papillomas, galactoceles, fibroadenomas, phyllodes tumors, malignant condition of the complex cystic and solid masses, invasive ductal carcinoma, ductal carcinoma in situ, and metaplastic carcinoma.

Conclusion
As the characteristics and accompanying findings of the complex cystic and solid masses, it is possible to subdivide categories and increase the accuracy of diagnosis.
