Background
Stroke is a leading cause of mortality and morbidity. Noncontrast Computed Tomography (NCCT) is the investigation of choice in assessment of acute stroke as it can help to identify early signs of stroke and can help rule out hemorrhage. A 10-point quantitative topographic CT scan score, the Alberta Stroke Program Early CT score (ASPECTS) scoring system is used to assess early ischemic changes.

Educational Goals / Teaching Points
To study the effectiveness of the ASPECTS scoring in the assessment of the anterior and posterior cerebral circulation stroke. For standardization of the reporting system using ASPECTS in patients with clinically suspected stoke.

Key Anatomic/Physiologic Issues and Imaging Findings/Techniques
We performed a retrospective study at our center involving patients referred to NCCT with clinical diagnosis of acute stroke. Patients were excluded with intracranial hemorrhage or hemorrhagic transformation of infarct. ASPECTS for anterior and posterior circulation was evaluated separately.

Conclusion
ASPECTS is an effective scoring system in emergency radiology practice with accuracy in delineation of infracted areas by easy visual mapping and better standardization. It also provides for easy and efficient communication with the treating physicians, which is very crucial in acute stroke and should be documented in every acute stroke report.
