E1946. Don’t “sLac” on the Nasolacrimal Duct: A Review of the Nasolacrimal Duct Drainage Apparatus and Associated Unique Pathology

The nasolacrimal duct drainage apparatus is an unfamiliar topic to many practicing radiologists, as many diseases are diagnosed clinically. Radiologic diagnosis can be challenging due to the complexity and lack of familiarity. Radiologists, in conjunction with clinicians, play an integral role in the timely diagnosis of associated pathology. Therefore, it is vital for the radiologist to become familiar with nasolacrimal duct pathology. The goal of this exhibit is to review the anatomy and imaging findings of various types of nasolacrimal duct drainage apparatus diseases.

Educational Goals / Teaching Points
The purpose of this exhibit is to provide background of the relevant clinical presentations in which nasolacrimal duct drainage apparatus pathology commonly occurs by reviewing the relevant anatomy of the nasolacrimal duct drainage apparatus, in addition to familiarizing the reader with common lesions, their associated clinical presentation, relevant anatomy, and pitfalls in diagnosis.

Key Anatomic/Physiologic Issues and Imaging Findings/Techniques
This presentation will concentrate on infrequent pathology that may present in the day to day workload of a practicing radiologist. The main topics to be discussed will be a brief review of the nasolacrimal duct drainage apparatus, as well as identification of frequent congenital infectious and neoplastic lesions. Mimics such as dacryocystocele, dacryocystitis, and squamous cell carcinoma of the lacrimal gland will be discussed using a case-based approach. Furthermore, common pitfalls, follow-up imaging, and potential complications will be presented.

Familiarity of the relevant anatomy and common pathology is necessary to radiologic interpretation of the nasolacrimal duct drainage apparatus to help guide proper diagnosis and clinical management.
